Schedule Changes & Drop-Ins: We're super flexible! Just let us know if you need to change up the schedule, miss & make-up a day or two, or add unexpected drop-in day(s). You can even text us the day of!

Holiday/Waiver Days: If there's no school, there's no after school hula.

New hula steps are taught 2x/wk, thus, at least 2 classes/wk are necessary. Hula can be taken a la carte or with homework & activities added w/ 2 pick-up options. Wednesdays have 3 pick-up options.

Pediatricians will only be contacted for medical decisions if parents/guardians cannot be reached.

ALLERGIES / MEDICATIONS / MEDICAL CONDITIONS
TERMS & CONDITIONS

ATTENDANCE: I understand that I need to give notice via phone, text, or email if my child will be absent or tardy from after school hula/activities. My child and I understand that attendance is taken daily, and my child is to check-in at Room 22 no later than 2:30p (Mo/Tu/Th/Fr) and 1:00p (We).

MAKE-UP CLASSES: I understand that if my child is absent, he/she can do a make-up any day later that semester.

​

NON-OPERATIONAL DAYS: I understand there will be no after school hula/activities on state and school holidays, other "no-student" days (per the PCHES calendar), half days, intersessions, and during any unforeseen school closures (for extreme weather, threats, facility emergencies like flooding, loss of electricity, etc.).

​

SCHOOL CLOSURES: In the event of an unforeseen or emergency school closure, I understand that I or my emergency contact will need to pick-up my child as soon as possible, and that I may be informed of a new safer pick-up location that my child may be transported to if the conditions at the school become unstable or unsafe in any way.

​

SUBSTITUTE: I understand that every effort will be made to arrange for a suitable substitute if the after school instructor(s) get injured or become ill. If a suitable substitute is unable to be found, for the best interest of my child, after school activities will have to be canceled (and a refund will be credited to me for the cancelation).

​

ILLNESSES: If my child is noticeably ill (presenting with a fever, Covid symptoms, etc.), I understand that I or my emergency contacts may be asked to take my child home early, for their well-being and that of their peers.

​

PICK-UP: I understand that I must pick up my child no later than 5:30p, and that I will give notice by phone or text if I will be late. If my child is not picked up by 6:30p, I give consent for my child to be driven to a new safer pick-up location (i.e. nearby open public library or shopping center) if the school lot becomes deserted or otherwise unsafe.

MONTHLY TUITION: I agree to pay the monthly tuition costs by the first class of each month. I understand that if my payment is not made by the 7th of the month, a $10 late fee may be assessed. I acknowledge that after the 15th of the month, my child may not be able to participate in hula/activities until delinquent payment + late fee is received.

​

DISENROLLMENT: I understand that I can disenroll my child at any time with a simple notification by email to pcafterschoolhula@gmail.com. I acknowledge that a prorated refund of the prepaid month can only be issued to me if I disenroll my child before the 15th of the month.

​

RULES: My child and I understand that it is my child's responsibility to follow all rules, as they are there for their safety as well as that of their peers, and that continued disregard for rules may result in dismissal.

WAIVER, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT

I understand that my child's participation in the after school activities offered by Pearl City Afterschool Hula, LLC is not without known and unknown risks, and that this agreement applies to all instances of my child's participation.

​

I understand that Pearl City Afterschool Hula, LLC is in full compliance with all school and state liability insurance requirements, and that they have provided the school with their Certificate(s) of Insurance per state regulations.

​

I understand and allow for the group recording/photography of my child engaged in hula and after school activities for the password-protected parent gallery and online/print sharing by Pearl City Afterschool Hula, LLC and/or Pearl City Highlands Elementary, without compensation, hereby releasing interest in said media for which I'm giving permission.

​

I understand that the State of Hawaii and Department of Education requires liability waivers and indemnification agreements from the participants of Pearl City Afterschool Hula, LLC's after school activities that take place on Pearl City Highlands Elementary School grounds, as applied to all instances of my child's participation in said activities, as well as any related off-campus measures that may be taken in regard to my child's safety or well-being.

​

Indemnification Statement: The undersigned individual, family members, personal representatives, agents, and successors, for and in consideration of the State of Hawaii Department of Education permitting and allowing the use of the designated school rooms, buildings, and/or facilities jointly and severally agree(s) to indemnify and save harmless the State of Hawaii Department of Education and Pearl City Afterschool Hula, LLC, against any and all loss, liability, demands, claims, suits, action or proceedings of every name, character and description which may be incurred by or brought against the State of Hawaii Department of Education and/or Pearl City Afterschool Hula, LLC for or an account of any injuries or damages to any person or property received or sustained by any person, directly or indirectly, by or in consequence of the participation of said activities and use of the school facilities/grounds.
